 Iowa law requires all incoming kindergarten and ninth grade students must provide the school with proof of a dental check-up/screening. The purpose of this law is to improve the oral health of Iowa’s children. For elementary school age children, this form can be
completed by a dentist, dental hygienist, physician, physician assistant, or nurse. For high-school, the form must be completed by a
dentist or dental hygienist only.
